PHOTO/MEDIA OPT-OUT 

From time to time, St. Francis of Assisi School may feature class or group photos or videos of students, quotations attributed to students, artwork or projects created by students in printed materials and in content on the school's website or school-controlled accounts on social media sites such as the school Facebook page and/or the school Instagram account. St. Francis of Assisi School agrees that students will not be identified by personal details other than first name.  

Consent and release are given without coercion or duress.  The photo, video or student statements may be used in subsequent years.  If the Parent/Guardian and/or student DO NOT want to be published, they may say so by submitting this form (Media Opt-Out Form) at any time during the 2023-2024 school year.

By entering my signature below, I DO NOT CONSENT to my child(ren)'s name, likeness, and/or schoolwork being used in connection with St. Francis of Assisi School as noted below:
