PART 1:  VIDEO OPT-OUT

During this time of hybrid instruction, St. Francis of Assisi School will be utilizing online resources to provide continuing classroom instruction to students.  In order to ensure that every child in the class receives the instruction, the online instruction will be video and audio recorded so that it can be accessed at a later time.

The primary focus of the video and audio recording will be on the instruction and the teacher, not on the students in the class.  However, questions and comments asked and spoken by students will be part of the recording.

Only students assigned to classes in which that specific class content is being taught will have access to the video recording.  Otherwise, the recordings will not be made public by the online vendor or St. Francis of Assisi School in any capacity.  This release form will be used to document your response regarding these activities.

I am the parent/legal guardian of the child(ren) listed above.  I have read and understand that online instruction will be subject to video and audio recording and as such, video images and/or audio recordings of my child might be captured.

By entering my signature below, I DO NOT GIVE PERMISSION for the video and audio recording of my child(ren) for online instruction.

PART 2:  MEDIA OPT-OUT 

From time to time, St. Francis of Assisi School may feature class or group photos or videos of students, quotations attributed to students, artwork or projects created by students in printed materials and in content on the school's website or school-controlled accounts on social media sites such as the school Facebook page and/or the school Instagram account. St. Francis of Assisi School agrees that students will not be identified by personal details other than first name.  

Consent and release have been given without coercion or duress.  The photo, video or student statements may be used in subsequent years.  If the Parent/Guardian and/or student DO NOT want to be published, they may say so by submitting this form (Media Opt-Out Form) at any time.
